Low T Center, a national leader in hormone replacement therapy and men’s wellness, is expanding its model to include increased hybrid care: clinic-based patient visits combined with telehealth appointments conducted onsite from within the clinic. We are seeking fitness-conscious, wellness-oriented Nurse Practitioners and Physician Assistants who excel in a fast-paced, customer-service-driven setting.

As a Hybrid Mid-Level Provider, you will work closely with the Physician Medical Director and the Center Director to deliver high-quality care across hormone replacement therapy, sleep apnea, weight management, hypertension, low libido, and other wellness services using our proprietary protocols.

What You’ll Do

• Provide both in-clinic patient care and telehealth visits performed onsite
• Evaluate, diagnose, and manage patients following evidence-based and proprietary treatment plans
• Collaborate with the Physician Medical Director and Center Director to support clinic operations
• Deliver an exceptional patient experience rooted in education, communication, and service
• Assist with clinic workflow, patient flow, and coordination of care
• Maintain accurate, timely documentation in the EMR system

What We’re Looking For

• Active Indiana Family NP or non specialty PA license in good standing (unencumbered)
• Current DEA and Current Controlled Substance License
• Technologically confident and comfortable with telehealth platforms
• Strong clinical decision-making and patient-communication skills
• Ability to provide care independently while also thriving in a team-based model
• Available to work 2 Saturdays per month (8am–12pm) per month
• Willing to float between clinics Occasionally
• Must be able to complete 10 consecutive weekdays of training (2 full weeks)

Training

We provide two full weeks of paid training to ensure your success in our clinical model, protocols, and systems.

Compensation

• $130,000 annually
• $400 per Saturday shift (4 hours) 2 required per month

Clinic Hours

Monday: 8:00 AM – 5:00 PM
Tuesday: 8:00 AM – 7:00 PM
Wednesday: 8:00 AM – 1:00 PM
Thursday: 7:00 AM – 5:00 PM
Friday: 8:00 AM – 5:00 PM
Saturday: 8:00 AM – 12:00 PM
Sunday: Closed
(Clinic closed daily from 1:00 PM – 2:00 PM for lunch.)
